REGISTRATION OF RIDER AND AGREEMENT PURPOSE: In consideration of the payment of a fee and the signing of this agreement, I, the parent or legal guardians thereof if a minor, do hereby voluntarily request and agree to participate in riding instruction as a student at Alpine Ridge Farms LLC, and that this student will a school horses provided by Alpine Ridge Farms LLC for instructional purpose, today and on all future dates:

$25.00 REQUIRED NON-REFUNDABLE DEPOSIT

Your spot will not be reserved until we receive your $25 non-refundable deposit.

$70.00 balance due the morning of the camp date.

1. I expressly agree and promise to accept and assume all the risks existing in this activity. The participation in this activity is purely voluntary, and I have elected to allow the above rider to participate despite the risks.

2. I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless Alpine Ridge Farms, LLC from all claims, demands, or causes of action, which are in any way connected with my participation in this activity or my use of Alpine Ridge Farm’s equipment or facilities, including any such claims which allege negligent acts or omissions of Alpine Ridge Farms.

3. Should Alpine Ridge Farms, LLC or anyone acting on their behalf be required to incur attorney's fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.

4. I certify that we have adequate insurance to cover any injury or damage that may cause or suffer while participating at summer day camp, or else I agree to bear the costs of such injury or damage myself. I further certify that I am willing to assume the risk of any medical or physical condition the participant may have.

5. Under the Michigan Equine Activity Liability Act, an equine professional is not liable for an injury to or the death of a participant in an equine activity resulting from an inherent risk of the equine activity.

I, THE UNDERSIGNED, HAVE READ AND DO UNDERSTAND THE FOREGOING AGREEMENT, WARNING, AND ASSUMPTION OF RISK AND RELEASE AGREEMENT. I FURTHER ATTEST THAT ALL STATED FACTS ARE TRUE AND ACCURATE.
